The MSPH & EMSPH Prospectus is now available both in print and on the website. The prospectus includes the admissions form. You can download the prospectus in word format and can also view the prospectus online, and also downloads a admissions form in Word format (if you download the form, please be aware that you have to print it out after filling it in and mail it to us with the completed application).

You can also receive a printed copy of the prospectus either by visiting HSA and picking one up, or by sending a written request addressed to the Registrar with your correct, full address provided along with bank draft of Rs. 1100 in favour of Health Services Academy. In the latter case, the prospectus and admissions form will be delivered to you by mail. Write to:

Please make sure to send in requests for the prospectus well before the application deadline, which is August 18th , 2010.

The admissions form can be downloaded in Microsoft Word format (requires Microsoft Word 2003). Please note that you cannot apply online; the application form must be downloaded, filled out, printed, and attached to the completed application to be sent to HSA by mail.

The downloadable admissions form is designed to be easily editable. Each entry field is grayed out. Please click inside the grayed out field, and start typing. The grayed out field will be replaced by the text you type, as shown below:

Step 1: Click on the grayed out field to select it.

Step 2: Start typing over the selection.

Step 3: Complete and print out the form.

Fill out the document. Save your document and print it out when you have completed the form. Attach this to your final MSPH & EMSPH admissions application.
